Hulk pile is Nomads en-Kor, Cephalid Illusionist, and Volrath's Shapeshifter.
Unless mentioned otherwise, "mill until" refers to milling until the condition is met, and resolving the Ulamog trigger whenever you mill Ulamog (This is the nondeterministic part).
Hulk Loops:

Mill until you mill Phantasmal Bear before Ulamog, putting Phantasmal Bear on top of grave.
Target Volrath's Shapeshifter with Nomads en-Kor.
In response, mill until you hit Hulk with Ulamog still in library.
Let the Phantasmal Bear trigger resolve, getting you a Hulk trigger.
In response, if you want an etb, mill until you mill the etb creature before Volrath's Shapeshifter and Ulamog. Otherwise mill until you shuffle your library in.
Let the Hulk trigger resolve, grabbing Volrath's Shapeshifter + up to 3 cmc of creatures.
If you put an etb creature on top of your grave, Volrath's Shapeshifter will enter as it and give you the etb.
This puts you where you started with an etb and/or some more creatures.

Hulk loop for Derevi etbs, making infinite mana.
Hulk loop to get Deathrite Shaman and Torch courier into play.
Haste DRS
Hulk loop to get infinite DRS untaps. Win by draining life with DRS.
