Update after playing this: The deck worked better as a Tiny Leader deck. In duel commander, the games I won have mainly been due to agressive starts with both Anafenza and random creatures. If i'd play this again, I probably would change it to a much more beatdown oriented deck, probably with the combo still included somewhere but much less focussed on it.

Melira, Sylvok Outcast, Vizier of Remedies and Solemnity make Kitchen Finks and Safehold Elite persist forever. With the sacrificy outlets Ashnod's Altar, Phyrexian Altar and Viscera Seer, we have an infinite loop and either infinite mana or scry through our whole deck. Blood Artist wins the game in that infinite loop, Grim Haruspex allows us to draw our library. Walking Ballista is lethal with infinite mana (unless we have Solemnity on the table, as it prevents the +1 counters). Duskwatch Recruiter finds the creature we need with infinite mana.

Chord of Calling and Green Sun's Zenith help us find our combo pieces.

Eternal Witness rebuys combo pieces from the graveyard.

Mother of Runes and Inquisition of Kozilek protect our combo pieces. Finally, there is some ramp and a bunch of removal to hopefully make it to the point where we can combo off.
