I just realized I didn't actually give you the list of reasons, so here it is.
Venser's Journal
This is just a bad card for this deck, you aren't drawing enough cards to need the handsize, and you shouldn't really need the life either (and it isn't that much).  You have better things to do with 5 mana.
Heartstone
I have used this card a lot... it has never worked out well for me.  It helps everyone and usually everyone else first.  It might be worth it with Sliverqueen, but she isn't your commander and will be hated on.  You are better off with something else here.
Library of Leng
Why so much hand protection? This isn't a great card and nothing in this deck looks to want it particularly.
Exquisite Blood
Again, this just feels out of place in this deck, if you want lifegain there are better options, and you aren't comboing with it, but everyone will assume you are and hate you out.
Anointed Procession
You aren't making that many tokens, doesn't seem worth it for this deck.
Chromatic Lantern
Your mana base is complex enough that you shouldn't need the ability to tap for any color and it just encourages you to get lazy and tap/fetch wrong then have it destroyed and end up lacking a critical color.  Use Coalition relic or other 3 mana stones instead.

If you are looking for cards to cut, I would cut Venser's Journal, Heartstone, Library of Leng, Exquisite Blood, Anointed Procession and Chromatic Lantern.

I would add Mana Acceleration cards, Look for things that cost 1 or 2 and add a mana or more every turn (Signates, and green fetch spells are going to be the big ones here).  3 to 5 of these should be a good number, if you add the full 5 you could even cut a land or two.

Speaking of cutting lands... my experience is that your mana base is too painful, even for EDH, and also needs more basics.  I would cut half the fetch lands and half the shock lands.  If you cut the enemy cycle of one and the ally cycle of the other that is probably the best way to do it, though you could cut the non-green fetches other than Arid Mesa and all the enemy shocks (green is most of your deck, so keep the green fetches and the non-green fetch that can get the green ally lands) to better fit your deck's colors.  Either way use that space to add 10 basic lands, two of each color is reasonable, one mountain and two forests might work better though.

Tribal soup

Set out to build a black/white rebel deck and ended up with a five color changeling deck... oops, looks like fun but haven’t taken it for a spin yet, anyone have suggestions or thoughts on it? Might add scarecrone or herald’s horn in place of ramosian captain, other than that the edhrec suggestions were fairly bad
