The deck is the classical Golgari reanimator deck built according to an optimized level.
Excluding a sort of "board control plan" aiming to reanimate creatures and destroy opponents' permanents continuously, the only win condition is infinite damages. The aim is to have on the battlefield Mikaeus, the Unhallowed and Triskelion. Triskelion enters with 3 +1/+1 counters on it and they can be removed to deal damage to opponents. When the last counter is removed, Triskelion dies and Mikaeus, the Unhallowed triggers: Triskelion gets undying ability and re-enters the battlefield as 2/2 creature with 3 +1/+1 counters. In order to repeat the aforementioned trick, now it is necessary to deal 2 damages on Triskelion (that will become a 3/3 creature with 2 damages on it) and 1 to an opponent (in this way, Triskelion dies because it is a 2/2 creature with 2 damages on it). After Triskelion death, Mikaeus, the Unhallowed triggers again reanimating it. Repeat the process and win the game.
The deck is a good-stuff deck including the most powerful green and black tutors (Demonic Tutor, Vampiric Tutor, Worldly Tutor, Finale of Devastation, Green Sun's Zenith, Survival of the Fittest), annoying creatures aiming to control the threats on the board (Demon's Disciple, Plaguecrafter, Fleshbag Marauder, Reclamation Sage, Caustic Caterpillar, Manglehorn, Spore Frog, Yawgmoth, Thran Physician), and useful card draw engine (Viscera Seer, Grim Haruspex, Phyrexian Arena, Sylvan Library, Liliana, Dreadhorde General, Skullclamp). Although it is not a fundamental piece in the deck, that can work fine without its commander, Meren of Clan Nel Toth provides a consistent reanimator booster every time a permanent dies or is sacrificed: entire deck strategy relies on sacrifice abilities (Diabolic Tutor, Eldritch Evolution, Phyrexian Tower, High Market, Fiend Artisan, Razaketh, the Foulblooded, Sidisi, Undead Vizier, Phyrexian Altar, Ashnod's Altar) to increase the number of experience counters in order to reanimate pieces, including the combo ones. Indeed, the two pieces of combo often are reanimated from graveyard (Reanimate, Necromancy, Animate Dead) after being discarded from deck such as they would be normally tutored (Entomb, Buried Alive, Unmarked Grave).
As said before, the deck should win with a control plan more related to ETB of creatures or trigger abilities of enchantments (Gray Merchant of Asphodel, Sheoldred, Whispering One, Dictate of Erebos, Grave Pact). However, generally this strategy is sub-optimal due to its inconsistency and lack of strong effects creating huge advantages (this is typical of stax decks).
Finally, the main weakness of the deck is always represented by grave-haters that destroy the reanimator synergies completely.
